Abstract:
The increased risk of cognitive dysfunction in older adults and the growing size of the elderly population with cognitive impairment can result in serious social problems. This study estimated the life expectancy with cognitive impairment of the elderly in Chins by employing multi-state life table method and using data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) in 2002-2018. We calculated the nodes of the simplified version of the MMSE in CLHLS for the classification of cognitive status in older adults by comparing with the standard version of Mini-mental State Examination (MMSE). The cognitive status of the elderly was classified as cognitive impairment and not, the former including mild/moderate/severe and dementia. The multi-state life table method was used to estimate the transition probability between the two states and to death in older adults. We also predicted the trend of cognitive impairment rates from 2019 to 2028. The study found: from 2002 to 2018, the life expectancy with cognitive impairment increased faster than life expectancy, supporting the hypothesis of " Expansion of Morbidity". The difference of life expectancy with cognitive impairment between males and females converged. From 2019 to 2028, the rate of cognitive impairment among the elderly would increase exponentially with age, and the rate of cognitive impairment of the female elderly would be higher than that of the males.

Qiang Li is a professor of School of Social Development, East China Normal University in Shanghai. She received her Ph. D from Institute of Population Research at Peking University and worked at Max Planck Institute for Demographic Research in Germany from 2005 to 2012. Her research involves the application of stochastic forecasting method to population in small areas in China, healthy life expectancy among older people and population ageing. She is currently secretary general of Shanghai Population Association.
